I am a social prescribing link worker based in Le Chéile FRC. Social prescribing is a free service that helps you to meet new people, take up a hobby or make a change in your life. Part of the Health & Wellbeing Community Referral Project, the process involves chatting together about what matters to you, and together coming up with a plan to bring about the changes you want.
